EFFICACY OF INTRAVITREAL ANTIBIOTICS IN TREATING ENDOPHTHALMITIS SECONDARY TO NECROTIZING KERATITIS: A CLINICAL CASE STUDY

Abstract

This study evaluates intravitreal antibiotic administration's efficacy in treating endophthalmitis secondary to necrotizing keratitis, illustrated through a clinical case involving an 81-year-old male patient. The patient presented with significant ocular symptoms, including vision loss, ocular pain, redness, and irritation, following the introduction of a foreign body into the left eye due to environmental exposure. Initial management led to inadequate improvement, prompting referral for surgical intervention. The surgical procedure included the administration of intravitreal vancomycin, along with thorough irrigation of the anterior chamber to remove purulent material. Post-operative assessment demonstrated a marked reduction in corneal infiltrate, improved anterior chamber clarity, and partial visual function restoration. These findings suggest that intravitreal antibiotic therapy, when applied promptly and effectively, can significantly improve clinical outcomes in cases of endophthalmitis associated with keratitis. This case underscores the necessity of timely intervention and appropriate therapeutic strategies in preserving vision and preventing severe ocular complications. The results contribute to the growing body of evidence supporting the use of intravitreal antibiotics as a standard treatment modality for ocular infections.
