PASTKI JAG‘ SINISHLARINI DAVOLASHDA ZAMONAVIY FIKSATSIYA USULLARINING KLINIK VA FUNKSIONAL SAMARADORLIGI

Abstract

Pastki jag‘ sinishlari yuz-jag‘ jarrohligi amaliyotida keng uchraydigan travmatik shikastlanishlardan biri bo‘lib, bemorning okklyuziyasi, chaynash funksiyasi, nutqi, og‘iz ochilishi va yuz estetikasi bilan bevosita bog‘liq. Davolash taktikasini noto‘g‘ri tanlash malokkluziya, infeksion asoratlar, suyak bitishining buzilishi, temporomandibulyar bo‘g‘im disfunktsiyasi va uzoq muddatli funksional cheklanishlarga olib kelishi mumkin. Ushbu maqolada pastki jag‘ sinishlarini davolashda qo‘llaniladigan intermaksillyar/maksillomandibulyar fiksatsiya (IMF/MMF), ochiq repozitsiya va ichki fiksatsiya (ORIF), titan miniplastinkalar, 3D-plastinkalar, locking va rekonstruktiv plastinka tizimlarining klinik hamda funksional samaradorligi adabiyotlar asosida tahlil qilindi. Tahlil natijalari shuni ko‘rsatadiki, oddiy va minimal siljigan sinishlarda yopiq davolash va IMF/MMF yetarli bo‘lishi mumkin, siljigan, beqaror yoki ko‘p fragmentli sinishlarda esa ORIF va barqaror ichki osteosintez anatomik repozitsiya, erta funksional tiklanish va asoratlar xavfini kamaytirish nuqtai nazaridan ustunlik beradi. 3D-plastinkalar ayrim holatlarda ko‘p yo‘nalishli barqarorlikni ta’minlasa-da, ularni universal yechim sifatida emas, balki sinish lokalizatsiyasi va biomekanik talablar asosida tanlash maqsadga muvofiqdir. Maqolada davolash usulini tanlash bo‘yicha amaliy klinik algoritm ham taklif etildi.
